You're Registered! Now What? (PDF)
This form will assist you in preparing your child for his or her stay at Camp Omega. It contains important packing, registration, and Canteen information as well as a map to camp. Please take time to carefully read and share this information with your child. Following these suggestions will allow campers to be prepared and have a great camp experience.

Health History Form (PDF)
A signed Health History Form is required for all summer camp participants and is due 7 days prior to the start of the camp session. You can also fax it to (507) 685-4401.

Summer Camper Information Sheet (PDF)
Please take time to complete this information sheet. It will help us know, understand and better provide and care for your child during his/her stay with us. This completed form is due along with the Health History Form 7 days prior to the start of the camp session. You can also fax it to (507) 685-4401.

Extreme Outpost Permission Form (PDF)
A signed permission form by a guardian or parent is required for Extreme Outpost campers to participate in the offsite high-ropes course.  Please print off, sign, and mail or fax prior to the camp session.

Summer Camp Evaluation (PDF)
One of the most important aspects of our summer program is the input from our campers and their parents so that we can continue to improve the camp experience at Camp Omega. We are especially open to any input and comments that will help to make the coming weeks and summers even better. These evaluations are anonymous, so please be honest and open with your remarks. If you do wish for us to contact you regarding your comments, please indicate. Please print, complete, and return to us by mail or fax. Thank you.

For the Children Campership Fund
We are doing everything we can to assure that every child can attend Camp Omega regardless of economic status. Don't let finances separate your child or family from this life changing experience. Many churches and organizations have funds available. Please contact your church office for information.

Camp Omega also has a Campership program called "For the Children Fund" designed to make sure that no person misses out on summer camp due to finances. Camperships can only be applied toward the basic camp program costing $300 or less. Camp Registration must be received before Campership Application can be considered.
